Please help us monitor everyone’s health by completing your daily self assessment. Your participation in this check-in helps the DDSB protect everyone’s health. A daily self-assessment must be completed every day that you are reporting to work.
PLEASE READ THESE QUESTIONS CAREFULLY AS THE SCREENING CRITERIA HAS CHANGED
1. If you are experiencing any new or worsening symptom of COVID-19 as listed below (not related to other known causes or conditions you already have), you must stay home until:
- You receive a negative COVID-19 test result.
- You receive an alternative diagnosis by a health care professional, or
- It has been 10 days since symptom onset, you have no fever (without using medication) and at least 24 hours since symptoms improving/feeling better
COVID-19 Symptoms
- Cough or barking cough (croup), new or worsening
- Shortness of breath/difficulty breathing
- Fever and/or chills (over 37.8 deg C)
- Sore throat
- Difficulty Swallowing
- Decrease or loss of taste or smell
- Nausea/vomiting, diarrhea, abdominal pain
- Runny nose, or nasal congestion (not caused by seasonal allergies)
- Tiredness, feeling unwell or muscle aches
- Worsening of chronic conditions
- Headaches (unusual or long lasting)
- Pink eye
- Increased tiredness/fatigue
- Falling often (for older persons)
If you are required to self-isolate and/or receive a recommendation for testing, you must contact your Supervisor as well as Ability Management ShortTerm Absence shortterm.absence@ddsb.ca. Please follow your standard absentee reporting process in a timely manner.
2. If a member of your household is currently experiencing any new COVID-19 symptoms and/or waiting for test results after experiencing symptoms you must isolate at home.
All asymptomatic household contacts of symptomatic individuals are required to quarantine until the symptomatic household member:
- receives a negative COVID-19 test result, or
- is cleared by Public Health, or
- receives an alternative diagnosis by a health care professional
If the symptomatic individual tests positive or is not tested and does not receive an alternative diagnosis from a health care professional, the symptomatic individual must isolate (including from household members) for 10 days from symptom onset, and all household contacts must isolate until 14 days from their last contact with the symptomatic individual.

3. If you or anyone you live with have traveled outside of Canada within the last 14 days from the date of entry into a DDSB building or have cared for or had close contact with an individual with confirmed COVID-19 within the last 14 days, you must stay home and contact your primary health care provider, local public health or Telehealth to discuss your symptoms and/or exposure and seek advice on testing.
If you are required to self-isolate and/or receive a recommendation for testing, you must contact your Supervisor as well as Ability Management. Please follow your standard absentee reporting process in a timely manner.
4. In the last 14 days, have you received a COVID Alert exposure notification on your cell phone? You must isolate at home and can only return after being cleared by public health.

PLEASE CONFIRM THAT YOU HAVE COMPLETED YOUR DAILY SELF-ASSESSMENT BY FOLLOWING ONE OF THE OPTIONS BELOW:

IF USING OUTLOOK (IE. FROM A DDSB DEVICE, THROUGH THE BOARD WEBSITE ‘STAFF EMAIL’ LINK OR USING THE ‘OUTLOOK’ APP ON A MOBILE DEVICE):
- CLICK THE LINK BELOW WHICH WILL GENERATE AN EMAIL
- CLICK ‘SEND’ ON THE GENERATED EMAIL (WHICH WILL SEND YOUR ACKNOWLEDGEMENT)

IF YOU ARE USING A MOBILE DEVICE AND ARE NOT USING THE ‘OUTLOOK’ APP OR HAVE BEEN NOTIFIED THAT YOUR DAILY HEALTH CHECK-IN WAS NOT ACKNOWLEDGED:
- CLICK THE LINK BELOW WHICH WILL REQUIRE YOU TO LOGIN IN WITH YOUR BOARD USER NAME AND PASSWORD AND WILL THEN DIRECT YOU TO THE DAILY HEALTH CHECK IN
